This is to be a supplement to other methods of identifying wormholes such as WH Type ID, "Show Info", and Out-of-Game methods such as staticmapper.

Wormhole Images

These images show what a wormhole leading to the relevant class of w-space system looks like.  Ignore the outer rings, their colour can be affected by the background colour of the system you are currently in - the centre bubble looks the same for every wormhole leading to a certain class.

K-Space

The colours on a wormhole leading to K-Space will reflect the region of space to which it leads. 

Wormholes leading to hisec or losec are generally fairly easy to identify - gold ones go to Amarr space, grey ones to Caldari, green ones to Gallente, red ones to Minmatar.
